Definition:
Defined by Petersdorf and Beeson in
1961:
- 1. Fever > 38.3 °C on several occasions
- 2. Duration of fever 3 weeks
- 3. Uncertain diagnosis after one week of
study in the hospital
Refinements of the definition have been proposed:
- 1. Elimintaing in-hospital evaluation requirement (since outpatient evaluation has become more
sophisticated).
- 2. Expanding definition to include nosocomial
- neutropenic vs. HIV fevers that might
not be prolonged
Epidemiology:
Over the past century, the most common causes
of FUO have not Changed
Scientific and technologic advances have refined
the def.
Undiagnosed FUO’s dropped from > 75% in
1930’s to < 10% in 1950’s.
Since then, this number has been increasing
Epidemiologic Variables:
Geography : increased ease in travel during the 21st century resulted in transposition of previously geographically restricted illnesses.
Age: FUO’s in children are usually secondary to self limited viral syndromes, while pt’s > 65 yrs old usually have multisystem disease.
AIDS: vast majority are infectious, minority malignant.
Neutropenia: most frequently bacteremia or fungemia, confounding variables are underlying malignancy, drugs, blood products,possible allograft rejection.
