Objectives: The increasing of potential medical demand in China has threatened the health of the population,
the medical equity, accessibility to medical services, and has impeded the development of Chinese
health delivery system. This study aims to understand the mechanism of the increasing potential medical
demand and find some solutions.
Methods: We constructed a system dynamics model to analyze and simulate this problem, to predict the
influences of health policies on the actual percentage of patients not seeking medical care (adjusting the
quantity structure of hospitals and community health systems (CHSs), adjusting outpatient prices, and
adjusting the level of health insurance).
Results: Decreasing the number of hospitals, increasing the number of CHSs, and raising the proportion of
health insurance compensation would effectively increase the transformation of potential medical
demand. But currently, changes of the outpatient prices didn’t play a role in the transformation of potential
medical demand.
Conclusions: Combined with validation analysis and model simulation, we suggest some possible
solutions. The main factors causing potential medical demand are accessibility to medical services and
proportion of health insurance compensation. Thus, adjusting the number of hospitals and CHSs and
increasing the proportion of health insurance compensation should decrease the actual percentage of
patients not seeking medical care and accelerate the transformation of potential medical demand, which

چکیده

اهداف: افزایش تقاضای بالقوه پزشکی در چین سلامت جمعیت، کیفیت پزشکی، دست یابی به خدمات پزشکی را به خطر انداخته و مانع توسعه سیستم تحول بهداشت چینی را شده است. این مطالعه قصد دارد مکانیزم افزایش بالقوه تقاضای پزشکی را درک نموده و راه حل هایی برای آن پیدا نماید.

روشها: ما مدل دینامیک سیستم را جهت آنالیز و شبیه سازی این مسئله ، پیش بینی تاثیرات سیاستهای بهداشتی در درصد واقعی بیمارانی که به دنبال مراقبت های پزشکی نیستند ساختیم (تطبیق ساختار کمیت بیمارستانها و جامعه سیستم های بهداشتی ) CHSها)، تطبیق قیمت ها و نرخ ها هنگام ترخیص بیماران، و تطبیق سطح بیمه سلامت).

نتایج: کاهش تعداد بیمارستانها، افزایش تعداد CHSها، افزایش تناسب جبران بیمه سلامت می تواند به طور موثری انتقال تقاضای بالقوه پزشکی را افزایش دهند. اما در حال حاضر، تغییرات نرخ های ترخیص نقش چندان مهمی در انتقال تقاضای پزشکی ایفا نمی کنند.

نتایج: در ترکیب با آنالیز اعتبارسنجی و شبیه سازی مدل ما بعضی از راه حل های احتمالی را پیشنهاد نمودیم. فاکتورهای اصلی که تقاضای پزشکی را موجب می شوند در خدمات پزشکی و تناسب جبران بیمه سلامت دست یافتنی هستند. پس تطبیق تعداد بیمارستانها و CHSها و افزایش تناسب جبران بیمه سلامت باید درصد واقعی بیمارانی را که به دنبال مراقبت های پزشکی نیستند را کاهش داده و انتقال تقاضای پزشکی را تسریع نماید که در سیاست گذاری نگرانیهایی را پدید آورده است

In this paper, a new model is developed considering diversity of service in service centers location problem. It is assumed that different services can be provided at each service center. The model has three objective functions including: minimizing the sum of customers’ travel time and waiting time in service centers, balancing service loads among the given centers, and minimizing the total establishment costs of service centers and assignment costs of servers. Different number of servers can be assigned to each service center. Regarding the allocation of customers to the centers, each customer patronizes with respect to the distance to the service center, the attractiveness of each service center’s site for the customer and the number of located servers at the service center. Since the proposed model is of nonlinear integer programming type and is of high complexity on solving, two meta-heuristic based heuristics using particle swarm optimization (PSO) and variable neighborhood search (VNS) are proposed to solve the problem. Different sizes of numerical examples are designed and solved in order to compare the efficiency of the heuristics.
